• I am voluntarily choosing to receive lash services.

• I understand that results may vary based on my natural lashes, lifestyle, and aftercare.

• I confirm that I do not have any known allergies, infections, or conditions that would interfere with this service (or I have disclosed them prior to my appointment).

• I understand that proper aftercare is required to maintain results.

I agree to arrive with a clean face and understand that failure to do so may affect my service.

I acknowledge that this is a mobile service and agree to provide a clean, safe, and suitable environment for the appointment.

I understand that all services are non-refundable and that my deposit is non-refundable.

By proceeding with booking, I release Paschal Beauty | Lashed By Liz and the service provider from any liability related to reactions, sensitivities, or outcomes associated with the service.
